A connected-care client solution built by PrimeStack, bringing patient management, appointment scheduling, care plans, and a companion mobile experience into one coordinated product.
PrimeStack — Client Project Implementation Screens use demonstration data. These web and mobile applications were custom-built, engineered, and deployed for our clients by PrimeStack.
The challenge
Make it easier for care teams and patients to move between appointments, care-plan information, results, and conversations without losing the context of the next action.
Business context
The deployed application features a staff workspace for clinicians and care coordinators alongside a patient-facing mobile app. All patient names, clinical values, and conversations are demonstration content.
The proposed solution
The delivered product pairs an Angular staff portal with a Flutter patient app and an ASP.NET Core API. The interface components cover appointment preparation, video-visit controls, medication schedules, and care-team messaging.
Proposed architecture
- Production Angular and TypeScript web portal with separate patient, scheduling, care-plan, results, and administration areas.
- Production Flutter and Dart apps for iOS and Android with reusable appointment, medication, and messaging views.
- Production ASP.NET Core and C# API with explicit access checks, request validation, and audit-event recording.
- Production SQL Server data model for patient relationships, appointments, care plans, and communication records.
- Production Azure hosting and private Blob Storage for documents; access policies and retention rules configured for the client.
- Video consultation integrated with a selected communications provider. Privacy, consent, security, and clinical review completed for launch.
